CUSTOMER INFORMATION UPDATE

Bank of Saint Lucia Limited appreciates that our customers are dynamic people. Over time you would have changed your address, telephone number or even employer. Help us remain in touch with you by informing us of those changes.

Simply print and complete the Customer Update Form and return to: CIF Project, Bank of Saint Lucia Limited, P.O. Box 1860, Castries, Saint Lucia.

We ask you to exercise caution in returning the forms so as to maintain the confidentiality of the information.

If you are resident overseas it would be necessary to have your signature witnessed by your Bankers or notarized and sealed by an Attorney. However, if you reside in St. Lucia, we recommend that you sign the form in the presence of a Bank of Saint Lucia Limited officer. Where there has been a change in your name, we would require a notarized copy of the document evidencing same. These measures are designed to protect you, as we must ensure that our records are changed based on verifiable documentation.
